Lisa Lorelli

Lisa Lorelli, BSN, RN, is the Director of Clinical Initiatives for The Primary Health Network. Lisa now oversees the Annual Wellness Visit and Collaborative Care Programs, and is preparing to introduce a Chronic Care Management pilot program to PHN. Additionally, she performs Annual Wellness Visits for the Tri County office in Cherry Tree, PA.  In March 2019, she began the journey in becoming a recognized participant in the Age-Friendly Health Systems by implementing the 4 Ms framework through the Annual Wellness Visit program.
