The courses are intended to support the unique health goals and priorities that matter to you, your loved ones, and the people you care for. You will be introduced to the 4Ms of Age-Friendly Care: What Matters, Medication, Mind, and Mobility. You will discover how each component of care relates to one another, learn tips on how to talk with your healthcare team about what’s most important, and gain access to resources that support health and well-being as we age. My 4Ms Worksheet & Video

The Age-Friendly Care, PA team has created an easy-to-use tool to help you get started with the 4Ms. First, spend some time considering your health goals and care preferences. Next, complete the My 4Ms Worksheet by identifying What Matters most to you. List concerns or questions about your medications, describe ways you relax and stay positive, and set an achievable daily mobility goal. Keep a copy of your 4Ms worksheet on your fridge and update it as needed - your 4Ms may change over time.
