Joy Weakland

Joy Weakland, LPN, employed with the Primary Health Network since 2014.  Her past positions with PHN were as a health coach which included educating patients about diabetes management, health hygiene and medication management. She has over 20 years of experience with the geriatric population which included skilled facilities and personal care homes. Her experiences vary in both clinical and management positions. Joy’s role on this project is champion to her site. Her passion is to help with the geriatric community and have them thrive in their everyday life.
